How do I find out if I can receive nutrition services?
- If you need nutrition services for a child with disabilities, contact your local DHHS office
- If you are (or need nutrition services for) an adult 18-64 years old with disabilities,
contact your local Center for Independent Living or League of Human Dignity
- If you are (or need nutrition services for) an adult aged 65 and above, contact your
local Area Agency on Aging
Do I have to get my doctors approval?
No, you and the nutritionist will determine your need for the service.
Do I have to go to the hospital or doctors office for this?
No, the nutrition services can be provided in your home or another place you choose.
