You Will Need:
Your Social Security number for a profession
MyLicense number (found on your renewal notice) for a facility
Mastercard or Visa (Debit or Credit)MILITARY: If you have served in the regular Armed Forces of the United States, during part of the twenty-four (24) months immediately preceding the biennial renewal date, and wish to waive the renewal fee, you must contact the Licensure Unit by email at rita.thalken@dhhs.ne.gov or by calling Rita at (402) 471-4927 BEFORE PROCEEDING.
