How to file a complaint

The Department investigates complaints regarding Long Term Care Facilities. To register complaints, contact the Licensing & Regulatory Affairs Health Care Facilities and Services Complaints.

Due to their confidential nature, complaint allegations should not be sent via e-mail.

Facilities self reporting an incident may use their own report, or one of the following forms:
  Abuse, Neglect, or Misappropration
  Elopement
  Resident to Resident
  Accidents
  Injury of Unknown Origin
