Reinstatement

All licenses for reinstatement require a check for disciplinary action through NABP.
A license on Inactive Status may be reinstated for a fee of $178.00. You are required
to complete the forms Application for
Reinstatement of License to Practice Pharmacy from Inactive Status. You must attest to
the completion of 30 hours of ACPE-approved continuing education within twenty-four months
of date of application to reinstate. You cannot claim an exemption from C.E. when you
reinstate your license.
For a license on Non-Disciplinary Revocation Status, it may be reinstated as follows:
Within one year—fee is $178.00 + $35.00 penalty fee. Also requires
completed Application
for Reinstatement of License to Practice Pharmacy Within One Year of Non-Disciplinary
Revocation attesting to the completion of 30 hours of ACPE-approved continuing education
within twenty-four months of the date of application to reinstate. You cannot claim an
exemption from C.E. when you reinstate your license.
Over one year—fee is $178.00 + $75.00 penalty fee. Also requires
completed Application
for Reinstatement of License to Practice Pharmacy More Than One Year Following
Non-Disciplinary Revocation, attesting to the completion of 30 hours of ACPE-approved
continuing education within twenty-four months of date of application to reinstate. You
cannot claim an exemption from C.E. when you reinstate your license.  Two Affidavits of Personal Recommendation for Reinstatement, Two Affidavits of Professional Recommendation for
Reinstatement, and Board approval at their next meeting are required.
For licenses on Revocation, Suspension, or Limitation due to Disciplinary Action or
Voluntary Surrender in Lieu of Disciplinary Action—fee to reinstate is $75.00.
Also requires completed Application for
Reinstatement Following Revocation, Suspension, or Limitation Due to Disciplinary Action
or Voluntary Surrender in Lieu of Disciplinary Action, showing the completion of 30
hours of ACPE-approved continuing education within twenty-four months of date of
application to reinstate. You cannot claim an exemption from C.E. when you reinstate your
license. Two Affidavits of Personal Recommendation for
Reinstatement Two Affidavits of Professional Recommendation for
Reinstatement pre-reinstatement investigation, and Board approval at their next
meeting are required. |
