Licensed Practical Nurse-Certified (LPN-C)
Procedures to Apply for Certification
To apply for certification, you must submit the following:
Application.
- Official proof of completion of an approved course.
- Fee of $68 (fee is non-refundable).
- If active LPN licensure is in Nebraska, no action is
required to meet this requirement. If your primary state of residence under the
Nurse License Compact Act is
Nebraska and your Nebraska LPN license is inactive or lapsed, you must reinstate your
Nebraska LPN license. If your primary state of residence is a compact state other than Nebraska and you
have a multi-state LPN license in that state, please refer to the instructions on the NURSYS
License Verification Request Form. If your primary state of residence is a compact
state and you have a multi-state LPN license in that state and it is not covered by
NURSYS, please contact the state directly for Verification of Primary State. If you
primary state of residence is a non-compact state, you must apply for LPN licensure in Nebraska.
An applicant does not need to take any action to demonstrate successful completion of
the examination as the records are maintained in the department.
There is no reciprocity with other states for certification as an LPN-C. Because the
qualifications for certification and the activities permitted differ from state to state,
all applicants for certification as an LPN-C in Nebraska must take the course and
examination approved for Nebraska.
If you have any questions about your application for certification, please contact Mary
Ann Moore by e-mail at maryann.moore@dhhs.ne.gov,
