Emergency Medical Services

Out-of-hospital emergency care providers are first responders, emergency medical technicians, emergency medical technician intermediates or emergency medical technicians paramedics who respond to a perceived individual need for immediate medical care in order to prevent loss of life or aggravation of physiological or psychological illness or injury.

Requirements for Provider Certification

To qualify for First-Responder certification based on education you must submit:

  1. A completed pdf icon application;
  2. A copy of your birth certificate or current drivers license to show evidence of being at least eighteen years of age;
  3. A copy of a current Cardiopulmonary Resuscitation certification (CPR) that includes adult, child, infant, and 2-person CPR from:
  1. Proof of successful completion of, within the two years preceding the application, the Department of Transportation First Responder course; and
  2. Proof of successfully passing, with a score of 70% or above, the National Registry First Responder Written Examination and Proof of successfully passing, within one year preceding the application, the National Registry First Responder Practical Examination; OR

To qualify for First Responder certification based on reciprocity from another state you must submit:

  1. A completed pdf icon application and pdf icon Attachment A;
  2. A copy of your birth certificate or current drivers license to show evidence of being at least eighteen years of age;
  3. A copy of a current Cardiopulmonary Resuscitation certification (CPR) that includes adult, child, infant, and 2-person CPR from:
  1. A certification from each jurisdiction or state where applicant is currently or previously licensed/certified (pdf icon Attachment A);
  2. Documentation that his/her license/certification was based on standards that are substantially equivalent to those prevailing in the State of Nebraska (pdf icon Attachment A); and
  3. A copy of your National Registry Certificate. (The Board of Emergency Medical Services has set the National Registry examination as the minimum examination standard for Nebraska.)

Proof of successfully passing, within one year preceding the application, the National Registry of Emergency Medical Technicians Intermediate Practical Examination, which includes skill demonstration in Patient Assessment Trauma, Ventilatory Management Adult, Intravenous Therapy, and Random Basic Skills; or

To qualify for Intermediate certification based on reciprocity from another state you must submit:

Examination Requirements

  1. Successful completion of a state-approved course within the discipline being applied for within the past 24 months;
  2. A copy of a current Cardiopulmonary Resuscitation certification (CPR) that includes adult, child, infant, and 2-person CPR from:
  1. Submission of a completed National Registry application;
  2. If applying to take the intermediate or paramedic examination, you must also have a current state certification as either an EMT or intermediate.

Provider Renewal Information

  • All certifications issued will expire on 12/31 of the 3rd year after issuance.
  • The Department will send a renewal notice at least 90 days prior to the expiration date to the address of record for those certificate holders whose certificates expire on 12/31 of that year.
  • There are no fees required for renewal.

Renewal requirements:

To renew a certificate, the certificate holder must submit:

  1. The renewal notice;
  2. A copy of a current Cardiopulmonary Resuscitation certification (CPR) that includes adult, child, infant, and 2-person CPR from:
  1. Verification of obtaining their continuing education hours (See Table A below); or
  2. A copy of a current certificate for the appropriate level from National Registry of Emergency Medical Technicians; or
  3. Verification from the applicant’s service physician medical director or qualified physician surrogate that the applicant is qualified for renewal; or
  4. Verification of passing an assessment examination. Certificate holders may only take the assessment examination one time in order to utilize such examination for meeting the renewal requirements. They must achieve a minimum score of seventy percent. If they fail the examination, they will be required to meet one of the other requirements. First Responders and EMT’s need only take a written assessment examination. Intermediates and Paramedics must pass a written and practical skills assessment examination.

Profession / Discipline Continuing Education Hours Required for Renewal.
1st Responder 20 hours in the subject matter of the First Responder course.
EMT 30 hours in the subject matter of the EMT course.
Intermediate 30 hours in the subject matter of the EMT course as well as documentation of demonstrated proficiency in peripheral IV administration and endotracheal intubation in a clinical, out-of-hospital or educational setting.
Paramedic 60 hours – Of which, 30 hours is in the EMT course and 30 hours in the Paramedic course as well as documentation of demonstrated proficiency in peripheral IV administration, endotracheal intubation, and drug administration and cardiac skills in a clinical, out-of-hospital, or educational setting.

Recertification Information

For recertification after expiration for less than 1 year if you met the renewal requirements before the expiration date, you must submit:

  1. A completed pdf icon Application for Recertification;
  2. A copy of a current Cardiopulmonary Resuscitation certification (CPR) that includes adult, child, infant, and 2-person CPR from:
  1. Documentation of meeting one of the following:
  • Verification of obtaining their continuing education hours (See Table A above); or
  • A copy of a current certificate for the appropriate level from National Registry of Emergency Medical Technicians; or
  • Verification from the applicant’s service physician medical director or qualified physician surrogate that the applicant is qualified for renewal; or

  • A copy of a current certificate for the appropriate level from the National Registry of Emergency Medical Technicians; OR
  • A copy of a certificate of successful completion of an appropriate refresher course, which must have been completed within two years of the date the Department receives the recertification application.

For recertificatin for more than 3 years, first responders and EMTs must meet the same requirements as listed above for 3 years or less. Paramedics and intermediates whose certificates have been expired for more than 3 years must reapply following the requirements for initial certification for the appropriate level.

Requirements for Service Licensure

To qualify for licensure as a Basic Emergency Medical Service, you must submit:

  1. A completed pdf icon application for an Emergency Medical Service License submitted at least 90 days prior to when the service expects to commence operations;
  2. A listing of the names, certification levels & certification numbers of employees/members of your service;
  3. Your written agreement with a licensed basic life support service if the applicant does not own or lease an ambulance;
  4. A description or map of your service area; and
  5. A copy of the pdf icon EMS Model Protocols or copy of modifications your service has adopted.

Service Renewal Information

All service licenses issued will expire on 12/31 of the 3rd year after issuance. The Licensing & Regulatory Affairs will send a renewal notice to the address of record for the EMS Service. No fees are required for renewal. Services are randomly audited each year. If the word AUDIT is in the upper right hand corner of the renewal notice, the service will receive an inspection before 12/31. The purpose of the audit is to determine that the service meets requirements outlined in Chapter 12.

Renewal requirements:

  1. Completion of EMS Basic Service Renewal Notice.
  2. A list of the names of active members of service identifiying each member’s certification.
  3. If applicable, prepare service records for audit.

Duplicates/Reissue

Duplicates: A duplicate large license or small license card may be requested at the time of initial licensure. A $10.00 fee, per document, is required.

Reissue: A large license and/or small license care may be reissued when the licensee requests: additional documents; replacement of original document(s) due to loss, mutilation or destruction; or replacement of document(s) due to a name change.

To request a reissued license, you must submit a pdf icon request for reissuance, and include a fee in the amount of $10.00 per document. Submit said request to:

Board of Emergency Medical Services

The Board of Emergency Medical Services promotes the dissemination of public information and education programs to inform the public about out-of-hospital emergency medical care and other out-of-hospital medical information, including appropriate methods of medical self-help, first aid, and the availability of out-of-hospital emergency medical services training programs in the state. The Board advises the Department of Health and Human Services Regulation and Licensure on all issues related to the regulation of emergency medical services. Board member composition and length of terms follows:

Membership and Requirements: 17 members appointed by the Governor, of which the composition must include:
  • 7 members shall be active out-of-hospital emergency care providers (2 first responders, 2 EMT’s, 1 intermediate, and 2 paramedics)
  • 3 members shall be qualified physicians actively involved in emergency medical care (at least 1 being a board-certified surgeon and at least 1 being a board-certified emergency physician)
  • 1 representative of an approved training agency
  • 1 physician assistant with at least 5 years of experience and active in out-of-hospital emergency medical care education
  • 1 registered nurse with at least 5 years of experience and active in out-of-hospital emergency medical care education
  • 2 consumers who have been residents of the State of Nebraska for at least 5 years
  • 2 members who have any of the qualifications listed above
  • In addition to any other criteria for appointment, among the members of the board there shall be at least 1 member who is a volunteer emergency medical care provider, at least 1 member who is a paid emergency medical care provider, at least 1 member who is a firefighter, at least 1 member who is a law enforcement officer, and at least 1 member who is active in the Critical Incident Stress Management Program. At least 5 members of the board shall be appointed from each congressional district. No more than 1 physician member shall reside in any single congressional district.
Length of Terms: All members serve staggered 3-year terms and may be reappointed.

The Board of Emergency Medical Services meets approximately 4-5 times per year – see Board meeting schedule for meeting dates and times. Meetings are open to the public, with the exception of investigational matters. Certification and Verification information can be obtained by checking the website at: www.dhhs.ne.gov/lis/lisindex.htm

To request either a certification or verification or your license to be sent to another Licensing & Regulatory Affairs/state licensing agency, send your request in writing along with the proper fee to the our office. Checks should be made payable to Licensing & Regulatory Affairs, State of Nebraska.

Certification

  1. Fee - $25.00
  2. Certification includes:
    1. Name
    2. Profession
    3. License number
    4. Issuance date
    5. Expiration date
    6. Whether the license was granted by examination, waiver, or reciprocity
    7. Examination information, if available

Licensee Assistance Program

The Licensee Assistance Program (LAP) is an assessment, education, referral, case management, and monitoring service designed to help licensees, certificate holders, and registrants of the State of Nebraska work through substance abuse/addiction problems.

The LAP services are free. However, in some cases treatment from a community resource agency may be needed. If further assistance is needed, the LAP Coordinator will help refer the Licensee to the most appropriate agency. When a treatment referral is made, those expenses become the responsibility of the Licensee.

This program was established by pdf icon Neb. Rev. Stat. § 71-172.02 and is funded by the addition of $1.00 per year to the current fee for each license issued, renewed, or reinstated.

Complaints

How do I file a complaint? To file a complaint, see the Investigations Division web site: www.dhhs.ne.gov/reg/investi.htm

What activities must be reported by a licensee, certificate holder, or registrant? Every credentialed person who has first-hand knowledge of unlicensed, illegal or unethical activities is required to report (includes self-reporting) within 30 days of the occurrence. Failure to report may result in discipline. To view additional information relating to mandatory reporting, click on the options below:

Mandatory Reporting shall not apply:

  • to the spouse of the person,
  • to a practitioner who is providing treatment to such person in a practitioner-patient relationship concerning information obtained or discovered in the course of treatment unless the treating practitioner determines that the condition of the person may be of a nature which constitutes a danger to the public health and safety by the person's continued practice, or
  • when a credentialed person who is chemically impaired enters the Licensee Assistance Program authorized by pdf icon Neb. Rev. Stat. § 71-172.01 except as provided in such section.
