Lead Based Paint Credentials
A person shall not be eligible to engage in lead-based paint activities unless the person holds a certificate issued by the Department of Regulation and Licensure, Licensing & Regulatory Affairs. The Department shall issue certificates to the following classes of lead-based paint professions: worker, supervisor, inspector, risk assessor, visual lead hazard advisor, elevated blood lead level inspector and project designer.
Requirements/Application Procedures
To apply for certification as a Lead Abatement Worker, you must submit the following to the Department of Regulation and Licensure, Licensing & Regulatory Affairs Division:
- A completed
application;
- The certification fee of $103;
- Two identical passport-sized clear, front view, full face photographs of you alone, recent enough to be a good likeness (normally taken within the last 6 months). The photographs should be 2 x 2 inches in size with an image of your head and shoulders taking up the majority of the area. Any retouching of photos is unacceptable;
- Documentation of successful completion of an initial lead abatement worker course, and all subsequent refresher courses, approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program with proof of passing the end of course examination. - A completed
physical examination form showing that you have been examined by a physician, within the preceding 12 months, and declared to be physically capable of working while wearing a respirator.
To apply for certification as a Lead Inspector, you must submit the following to the Department of Regulation and Licensure, Licensing & Regulatory Affairs Division:
- A completed
application;
- The certification fee of $203;
- Two identical passport-sized clear, front view, full face photographs of you alone, recent enough to be a good likeness (normally taken within the last 6 months). The photographs should be 2 x 2 inches in size with an image of your head and shoulders taking up the majority of the area. Any retouching of photos is unacceptable;
- Documentation of successful completion of an initial lead inspector course, and all subsequent refresher courses, approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program. To apply for certification as a Lead Risk Assessor, you must submit the following to the Department of Regulation and Licensure, Licensing & Regulatory Affairs Division:
- A completed
application;
- The certification fee of $203;
- Two identical passport-sized clear, front view, full face photographs of you alone, recent enough to be a good likeness (normally taken within the last 6 months). The photographs should be 2 x 2 inches in size with an image of your head and shoulders taking up the majority of the area. Any retouching of photos is unacceptable;
- Documentation of successful completion of initial lead inspector and risk assessor courses, and all subsequent refresher courses, approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program. To apply for certification as an Elevated Blood Lead Level Investigator, you must submit the following to the Department of Regulation and Licensure, Licensing & Regulatory Affairs Division:
- A completed
application;
- Two identical passport-sized clear, front view, full face photographs of you alone, recent enough to be a good likeness (normally taken within the last 6 months). The photographs should be 2 x 2 inches in size with an image of your head and shoulders taking up the majority of the area. Any retouching of photos is unacceptable;
- Documentation of successful completion of an initial elevated blood lead level investigator course, initial lead inspector course, and initial risk assessor course, and all subsequent refresher courses, approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program. Certification examinations:
- A certification examination will be administered by the Department of Health and Human Services, Regulation & Licensure. Contact Bobbie Mills at bobbie.mills@dhhs.ne.gov for information about the certification examination.
- Certification examinations are required to be certified as an: inspector, risk assessor, supervisor, elevated blood lead level investigator, and visual lead hazard advisor.
- Restrictions: No applicant for certification shall be permitted to take the certification examination more than three times within six months of receiving a program completion certificate from an approved training or review program.
- If an applicant for certification does not pass the certification examination within six months of taking the initial program, that he/she must retake the appropriate program.
- Study sheets are available upon request
- The certification examinations were developed by the Environmental Protection Agency (EPA).
End of course examinations:
- End of course examinations are administered by the training program after the training course is complete for the following lead professions: certified worker and certified project designer.
- The course and examination must be approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program.
These forms may be completed online, however, they cannot be submitted via e-mail due to the fact that all applications and forms require original signatures. If you complete the form online, you must print the form and mail it to the address below.

- All lead-based paint certifications expire three years from the date of issuance.
- Individuals may renew their certification any time up to one year after its expiration. After one year they need to reapply for initial certification.
- To renew a certificate, the certificate holder must submit to the Department of Regulation and Licensure, Licensing & Regulatory Affairs:
- the Renewal Notice;
- the renewal fee;
- documentation of successful completion within the preceding year of a lead-based paint refresher training course (within the discipline applied for) approved by the Department of Regulation and Licensure, Asbestos/Lead Based Paint Program with proof of passing the end of refresher course training examination;
- a completed physical examination form showing that they have been examined by a physician, within the preceding 12 months, and declared to be physically capable of working while wearing a respirator. Only workers and supervisors are required to obtain a physical examination;
- Renewal fees are the same as the initial certification fees.
- A renewal notice will be sent out to the last address of record at least 30 days prior to expiration.
As a condition of renewal of the certification, and before submitting the renewal notice to the Department of Regulation and Licensure, Licensing & Regulatory Affairs, the applicant must successfully complete an approved refresher course or approved initial program if applicable for the profession they are applying.

Duplicates: A duplicate large license or small license card may be requested at the time of initial licensure.
Reissue: A large license and/or small license card may be reissued when the licensee requests: additional documents; replacement of original document(s) due to loss, mutilation or destruction; or replacement of document(s) due to a name change.
