Service Coordinators meet with people with developmental disabilities and their families to develop plans for services that meet objectively assessed needs, and reflect individual choices and preferences.  Service Coordinators help manage services through direct monitoring, ensuring people’s needs are met.

Eligibility Determination

• Using a standard process, Service Coordinators determine eligibility for specialized services funded by the State of Nebraska.

Resource Coordination

• Service Coordinators connect people with non-specialized supports and services to meet their needs.
• Service Coordinators facilitate Individual Program Plan (IPP) Team meetings to develop integrated plans of services.
• Service Coordination authorizes funding for specialized services.

Monitoring for Satisfaction

• Service Coordinators monitor the services authorized in the IPP.
• Service Coordinators advocate for the rights of people receiving services.

The state-operated system, which is independent of the direct provision of supports and services, offers persons with developmental disabilities the following advantages:

• Avoidance of potential conflict of interest
• Access for all eligible individuals, regardless of disability, age or need
• Direct link to funding of services
• Consistent, statewide implementation of Service Coordination
