What’s a program?

A program is a group of services intended to meet a client’s or family’s need. The program helps people who are aged or have disabilities to meet their needs and achieve their goals in how to live and where to live.

First, a person must qualify for the program. Each program has its own requirements. Persons who are eligible for a program are called clients. Once eligible, clients will partner with a Services Coordinator or a Social Services Worker to get the services they need, and help achieve their goals.

Programs offered by Home and Community Services:

I need assistance for: A child with disabilities, An adult age 18-64 with disabilities or An adult over 65.
