Immunization Records
The administering immunization clinic or physicians office keeps an immunization record
on file for each person who receives an immunization at their facility. A personal copy is
given to the parents. Every time you visit your health care provider for immunizations,
its important to take your immunization record with you and have it updated. At this
time, Nebraska does not have a statewide immunization database so it's very important for
parents to keep their childrens as well as their own records.
The medical record contains the following immunization information:
- Names
of the vaccine you receive;
- Which
dose in the vaccine series youre getting;
- The
signature of the person authorizing the vaccination for self or the child;
- How
old child is when he/she received the immunization;
- The
manufacturer's name and lot number of the vaccine being given;
- The
date the immunization was administered;
- The
site (arm or leg) where the vaccine was given;
- The
signature of the nurse administering the immunization;
- The
appropriate authorization to release information to physicians, schools, etc.; and
- The
name of the current health care provider;
The immunization record is the document used to verify immunization for school entry
and day care enrollment. Because memory can be unreliable, only an immunization record is
accepted. If a child has no immunization record, he/she may need to begin the immunization
series all over again.
