Commercial businesses that provide passive devices and operate laboratories to determine indoor radon concentrations are regulated by the Department of Health and Human Services.
As explained in
180 NAC 11 (Nebraska Regulations for Control of Radiation), "... no person may provide services for the measurement or mitigation of the presence of radon in the State of Nebraska unless such person has been licensed as provided in this section."

The form is provided in two formats.
